Action and mechanism
Association antacids. Sodium bicarbonate is an absorbable antacid (sodium). Sodium derivative. It works by neutralizing the hydrochloric acid in the stomach, increasing gastric pH, which also reduces the formation and activity of pepsin.
- [Gastric hyperacidity]. symptomatic treatment of disorders related to stomach hyperacidity (heartburn, indigestion).
Oral. Grown ups:
-Molestias gastric hyperacidity associated with: 1 or 1 tablespoon of powder, 20 min-1 hour after meals or when discomfort arise, for a maximum period of two weeks without clinical control.
Children over 6 years old: half the adult dose.
- Standards for the correct administration: Dissolve the powder or the contents of an envelope in half a glass of water.
[RENAL]. In renal failure may occur an accumulation of sodium that can produce hypernatremia and increased fluid retention.
[HEART FAILURE]. Sodium can produce a hypernatremia to result in alterations in the Na + -K + ATPase pump, decreasing the effectiveness of the cardiac contraction. In addition, water retention can generate a hypervolemia to produce an increased preload and even more difficult heart functionality.
[ARTERIAL HYPERTENSION]. The sodium content of this medicine may increase fluid retention and blood pressure.
[ALKALOSIS METABOLIC], [Respiratory alkalosis]. Sodium bicarbonate is a systemic antacid which is absorbed in the gastrointestinal tract, so its use for a long period can result in metabolic alkalosis.
Warnings / Tips
Sodium salts, such as sodium bicarbonate or sodium carbonate, can produce a rebound by increasing acidity of gastrin secretion and gastric secretion in response to the large increase in gastric pH that occur.
This medicine may decrease the oral absorption of ketoconazole and tetracyclines as well as decrease the renal excretion of amphetamine and quinidine and increase the excretion of lithium carbonate (possible reduction of antimanic effect).
Adverse effects of sodium hydrogencarbonate are generally infrequent but may be moderately important if high doses or during prolonged treatment are used. - Gastrointestinal: Occasionally, [Flatulence], belching, vomiting, stomach cramps, stomach distension and heartburn as rebote.- Metabolic effect: With large doses can produce [ALKALOSIS METABOLIC] (more often if there renal failure), [hypernatremia] and increase serum lactate. If large amounts of milk ingested (high calcium ion) may result in a syndrome Milk-alkali, with hypercalcemia (anorexia, nausea, vomiting, lethargy, muscle cramps), which can manifest several weeks after initiation of treatment .- Cardiovascular: for sodium absorption, risk of [Edema] and [HYPERTENSION] with high doses or prolonged treatment.
